Applicant Membership Documents
Please review the required membership documents outlined below that will be requested for upload as the final step of your application process.
You may submit them now or return to the ATA website within 7 days to provide the documents. Please make note of the website login and password you will create,
you will need this in order to login to the system later.
Once the documents are received, your application process will be complete and sent to the ATA Membership Panel for review/recommendation.
Active or Corresponding Members:
1. Curriculum vitae.
2. In one document, please provide a brief description of how your current professional activities reflect your interest
in clinical thyroid disease or thyroid-related scientific investigation as well as, a brief description of what motivates you to join the ATA
(50 words or less for each description.)
Associate Members:
1. Proof of fellowship letter from your program director which should include your fellowship start date and expected completion date.
2. In one document, please provide a brief description of how your current professional activities
reflect your interest in clinical thyroid disease or thyroid-related scientific investigation as well as, a brief description of what
motivates you to join the ATA (50 words or less for each description)
